Member Category Change

I hereby agree: 1) to a change in membership category from ESP to Teacher and 2) to pay dues for the member category and in the corresponding amount indicated above, retroactive to the initial date of my change in membership category. I authorize WEAC or MTEA, to adjust the amount to be charged to or debited from my account to satisfy the dues amount change resulting from my change in membership category by adjusting my payments equally over the payment schedule noted on my initial membership application.
